Capturing Name and Address Information When Using a Saved Button

I'm trying to create an HTML membership form for a volunteer organization.  Basically all it needs to do is collect name/address/email/etc. and then collect the money using Paypal.

I've created saved buttons and can invoke them from my form with no problem.  However I don't see any way to pass the member name information so that I can co-relate the member to the payment.  So I can see the payment but not who paid it and, in the case of new members, don't have any contact information.

I've checked developer.paypal.com and see code snippets and form variables but none of them see to allow me to do that.
