buyer wants a refund, while item is in transit. what should I do?

Hello, I recently sold an item to a buyer, I shipped the item to the buyer. They live in a different country and "the item is taking to long to get to them and wants a refund". What should I do? do I have to give them refund before they even get it or send it back? Who's side will PayPal take? thanks for the help
