Is paypal account free to create it or have to pay for it

Free, you only pay fees if you are receiving goods/services payments / withdrawals in some countries to a card OR / friends/family payments (if available in your country funded via a credit card).

When I was trying to add my card I got the message same time was stating my bank declined the transaction of usd 1.00 at paypal declined

That's the card confirmation process which is refunded as soon as you enter the 4 digit number sent to your card account to confirm it as per link.
