Welcome to the PayPal Community! This is a great topic. First we'd recommend requesting a Name Change through PayPal.com.
Second, is this a PayPal Debit Card or a PayPal Prepaid Debit Card? If it's a PayPal Debit Card, you can Request a new one here. If it's a Prepaid Debit Card, you'll want to update your name and request a new card by reaching out to the PayPal Prepaid Customer Support teams. They should be able to get you all sorted out.
