Bank account confirmation during signing in to Paypal

Two small amounts supposed to be paid by Paypal to cofirm my bank account not showing in the specified boxes. Why is that? Waited for weeks now! Please help...

What boxes?? The code is supposed to show on your bank account statement and then you put the code in the box.
