Collecting more information with the Add to Cart button

We have a cycling event we want to register people for using the "Add to Cart" button. We need to collect 4 additional pieces of information and I only see 2 boxes for collecting this info with the button. How do I get this additional information? I tried to add html coding once the button was on our site, but only 2 of the boxes had info come through.

Here are the questions I need answered:

Age

Which version of the event to you want to do? 80 mile or 110 mile

Emergency Contact & Phone #

Do you accept the liability release?

Using the online button creator, Buy Now item button are limited to 2 options, Add to Cart item buttons are limited to 10 options.
