I am having this problem in my local development enviornment, But Some of our client told us the same problem is also occured in the live enviornmet for few countries only, but the problem is we dont know for which countries this issue occured.

Is this related to the 500 server errors we keep getting in the production environment?

I wouldnt so, the original issue looks to be related to uncorrectly formated address data being passed.
