account hacked unauthorized transaction. Anybody else experience this issue?

I live in Washington & my account was hacked in Florida with  in store purchase. How can this be possible with my card right here in my hand? How did they get my card numbers & my 3 digit code or password's? Any help would be greatly appreciated.
