Need help submitting chargeback

Hi there. I need help with submitting a chargeback claim. 

 

I input everything I have, evidence and all, and go to submit it but I just get a "Sorry, we may have had a problem on our end, please try again" from PayPal. Then it all erases and I have to redo it. I've tried 3 times now and I keep getting the same "Tray Again" message. Can anyone give me a hand with this please?
