Date Error with "Report a Problem"

Someone else already expressed running into this issue. I'm trying to file a chargeback. When I get to the page where it asks "did you cancel the order" and fill in the information, it does not detect the date and gives me an error
"Some required information is missing or incomplete. Please try again."
"Please enter a date"
I tried from different browsers, Safari, Google Chrome, Safari from my MacBook but nothing changed. Please let me know if there is a solution to this. Thank you.
