Account hacked, unauthorized payment to unknown account

After more than a decade on PayPal without issues, an unauthorized payment was made from my account today without my knowledge. I immediately submitted an appeal and just now PayPal notified me that my appeal was denied.
How can this type of unauthorized use happen, how can I get reimbursed, and why did PayPal not even contact me before denying my case.
This is outrageous as someone just stole almost $250 from me and PayPal doesn't seem to care?
What are my options now to get my money returned?

What is not considered an Unauthorized Transaction
The following are NOT considered Unauthorized Transactions:
- If you give someone access to your PayPal account (by giving them your login information) and they use your PayPal account without your knowledge or permission. You are responsible for transactions made in this situation.
- Invalidation and reversal of a payment as a result of the actions described under Refunds, Reversals and Chargebacks.
Advice is voluntary.
