So my sister made a payment in an app without asking me or permission and I need a refund, the purchase of that thing is still not used

You can contact the seller and ask if they will refund.

Or you can file an unauthorised dispute in the resolution centre BUT as it was you that allowed her access to your Paypal account and your account was not hacked its unlikely they will pay out.
