Card activated on another account?

I'm trying to add another credit card to my account, but it says that it has already been activated on another account--none that I am a aware of.

 

I would like to know if this is an error or an account that I somehow forgot about or someone stole my card, etc.

I would suggest giving our Customer Service a call.  They would be able to look up this information to see if you might have had a previous account.  No worries--they will certainly be able to help you out!
