Accepting payments w/ invoicing

Just a real quick question, and I probably skimmed over it while looking for this..

I want to accept credit cards on my website for monthly subscriptions (with a 1 month trial at the start). And I need invoicing as well. 
So does the 2.9% + $0.30 include the invoicing fee, or does it work like this:

Accepting Payments: 2.9% + $0.30 per transaction
PLUS
Invoicing: 2.9% + $0.30 per transaction.

Regardless of method, basic fees are 2.9% plus $0.30 per transaction.  Makes no difference if sending an invoice or using an item button.
