You can't withdraw to a card in the U.S.
You can withdraw to a linked bank account / get them to post you a cheque or use a paypal debit card to get it from an ATM.

open an account in Italy... that will work! Or Luxembourg... be creative with your address! But be careful with your IP-address - Paypal blocks some of them! Don't try to transfer more than 19 Euro per transaction, and not more than 1 per 24 hours - else your account will be <removed>!

What a ridiculous suggestion. Paypal is country specific so if you open a paypal account in another country you would not be able to add your U.S address or cards.
