Card declined for payment, no reason, several occasions

I was attempting to swipe a client's debit card and everytime I did it would go to the processing screen then to payment declined.. I tried entering it manually, double and triple checked all the info, and it still said the card was declined. This happened a month ago for the same value. I thought it was actually going through at first but it's not. Why can't I accept a payment with her card and how do I fix it?

a card can be declined if it has expired,reported stolen,exceeds its credit limit or just issued by some entity not in the net work and cannot be verified.
